We located a range of questionnaires that included questions relating to Ofsted criteria on 'healthy lifestyles', the extent to which pupils 'feel safe' and how they 'contribute to the school and wider community'. However, despite extensive research, we found no published examples of questionnaires specific to 'workplace and other skills'.
'How well are pupils doing?': sample pupil questionnaires
Primary pupils
Yacapaca is a website that enables the sharing of pupil voice resources. It includes a range of online questionnaires that relate directly to how pupils feel about their safety; whether they adopt healthy lifestyles; and how they contribute to the school and wider community.

This ‘every child matters’ (ECM) questionnaire is mainly about wellbeing. It is aimed at primary age pupils and includes questions on:

- General concerns pupils may have

A healthy lifestyle: sample pupil questionnaires
Leicestershire County Council has produced a range of ‘healthy schools’ resources, including a list of suggested questions to help measure aspects of a healthy lifestyle. The questions are in line with the 'how well pupils are doing' section of the 2009 Ofsted evaluation schedule of judgements for schools.
The list offers suggested questions for pupils, staff, and parents and carers. It is organised under:
- PHSE
- Healthy eating
- Physical activity
- Emotional health and wellbeing

Healthy eating: sample pupil questionnaires
Camden and Islington's healthy schools programme has a healthy eating questionnaire for pupils. It asks specific questions about eating habits at breakfast and lunch times. It also asks pupils about the drinks they consume every day.
It has a section with questions about fruit and vegetable intake and questions on general healthy eating issues.

Physical activity: sample pupil questionnaires
Leicestershire County Council has a physical activity questionnaire. It asks pupils about the amount of physical activity they undertake each week and the types of activity they partake in.

An exercise questionnaire, written by a 10-year-old pupil for his classmates, asks about attitudes to exercise, the amount of exercise pupils do each week, and perceptions of what motivates children to participate in sport. A couple of the questions are phrased in a slightly negative manner so it might be useful to re-phrase them.

A school travel questionnaire from Perth and Kinross Council includes questions on health and fitness. The questions ask about the types of activities that pupils participate in as well as the length of time spent exercising each week.

A school travel survey from the charity Sustrans includes questions on health and fitness. Pupils are asked about what kinds of activities they take part in, and how regularly they do so. It also asks how pupils feel about their health.

Leicestershire County Council has a questionnaire that focuses on feeling safe and secure. It asks pupils how they feel about adults in the school and about their classmates.
